RoleThe ideal candidate for this position will possess experience in working with individuals affected by substance abuse. This role will involve responsibilities working at both the Prescott Valley Community Health Center and the Connection Center in Prescott.
The Yavapai County Connection Center works with newly released inmates, specifically those impacted by substance use disorder, reintegrating into the community.
Bilingual: Spanish/English speaking preferred.
Under direct supervision, organizes and coordinates medical services and follow up for Community Health Center of Yavapai (CHCY) patients. This position works with practitioners and patients at all locations
Major Duties, Responsibilities- Works cooperatively with patients and practitioners to identify problems/barriers, appropriate actions/steps, and timelines for completing health care goals.
- Follows-up on no-show appointments of specifically identified patients and expedites and facilitates patient referrals to specialty care.
- Provides patient education as to the importance of following through on prescribed treatments. Works with community providers to ensure that referred diagnostic testing is scheduled and completed timely.
- Provides patient education regarding CHCY services and specific program eligibility.
- Monitors progress in achieving goals to demonstrate improvement in the care and clinical outcome of the patients receiving care coordination.
- Assists with coordinating local marketing initiatives including advertising, materials development, and distributions such as flyers and patient handouts. Helps plan and implement community events such as health fairs, group presentations, and professional education activities.
- Performs other job-related duties as assigned.
Minimum QualificationsEducation:High school diploma or equivalent.
Experience: No experience required.
Additional Requirements:Must possess a valid Arizona driver’s license.
Must possess a current fingerprint clearance card issued by the Arizona Department of Public Safety.

Work Environment:The work environment is associated with an office and healthcare facility. The work area is adequately lighted, heated or cooled and ventilated. While performing the duties of this job, the employee may be exposed to blood and body fluids and occasionally may be exposed to toxic or caustic chemicals.
Physical Requirements:Ability to sit or stand for extended periods of time. Sufficient vision to read printed materials and a computer screen; hearing and speech ability to communicate effective in-person and telephonically. Capable of frequent repetitive movement of arms, fingers, and wrists. Ability to lift up to 30 pounds.
