Role Description: This position is responsible for the switchboard and providing clerical assistance to the Support Coordination Team.
Preferred Skillset: Proficient in Microsoft Office Suite, time management skills and ability to meet deadlines; ability to work on multiple projects and prioritize task effectively; strong verbal and written communication skills; strong interpersonal skills and the ability to be adaptable.
What you’ll do: - Take, log, and transcribe weekly Support Coordination meeting notes and send to supervisor for approval/revisions.
- Log, track, and send to filing all monthly, quarterly, and annual service monitoring for support coordinators. Maintain tracking to help ensure all service monitoring is completed, sent out, and filed as required.
- Print and hand out all Support Coordinators in weekly team meetings reports of IHP coming due (120 days), obtain updates from all Support Coordinators and enter on spreadsheet.
- Provide the Support Coordinator supervisor the following reports prior to the first months SC team meeting: All Caseloads; HI's, coming due/expired; LOCs coming due/expired; Authorizations coming due/expired.
- Maintain Consumer ISP's, amendments, and other relevant correspondence.
- Print individual specific information form CIMOR (face sheet, guardian information, authorizations, NAFs balance) in advance of ISP meetings for Support Coordinators.
- Switchboard responsibilities include but are not limited to: answering the phone, transferring calls, taking messages, and face-to-face correspondence; sorting and distributing mail from various other agencies, and sorting and distributing mail form the main office.
- Assist clerical team as needed as work-flow fluctuates.
