    BEHAVIORAL HEALTH CASE MANAGER I/II

    Multiple vacancies with the Tulare County Health and Human Services Agency located in Visalia. This recruitment will establish an employment list to fill current and future vacancies in various programs and facilities.

    Vacancies are located at the Transitional Living Center (TLC) and the Visalia Adult Integrated Center (VAIC) both facilities are located in Visalia. Positions located at the TLC will require shift work including nights, weekends and holidays.

    This position requires incumbents to engage with consumers of Behavioral Health and/or Alcohol and Drug Services in reducing barriers to treatment and providing support to their wellness and recovery.

    Vacancies can be filled at either Level I or Level II depending on qualifications.

    SALARY INFORMATION BY LEVEL
    Level I: $3,441 - $4,194 Monthly
    Level II: $3,799- $4,630 Monthly

    Typical Duties

    Actively engages clients/family to determine what services and support will assist in reaching their personal goals, considering the client’s/family's unique needs and preferences; coordination and client linkage to community-based resources to maximize support systems, promote wellness and recovery, and minimize risk of hospitalization or homelessness; ensure that clients attend all necessary therapy, medical, and medication management appointments as outlined in their wellness plans by providing reminders, help with access to transportation or other resources needed to eliminate barriers to access services; ensure clients’ safety through various assignments, including but not limited to general observation in one-on-one or large group settings; ensure client/family receives or is connected to adequate support, resources, and education when client experiences a transition in settings including but not limited to returning to the home from a residential stay or incarceration to transitional housing; maintain productivity expectations related to activities that are billable and may be reimbursed by state, federal, and/or other payor oversight entities and as determined by program needs; conduct and document in-home evaluations and medication management, and present updates to a multi-disciplinary treatment team of initial results and ongoing status of wellness goals; build rapport with clients by listening, interacting with them, and engaging the client in treatment; observe and document client behavior, including staff interventions; manage a caseload by ensuring timely client contact, initiating and maintaining a variety of progress notes, service records, coordinating with other service providers when appropriate, and other required documentation in standard form and language; ensure timely submission of all documentation given expectations of program and supervisor; confer with and make referrals to higher-level staff members when problems and client’s needs are beyond his/her level of competency; consult with peers, multi-disciplinary team members, and supervisor, as appropriate, to ensure appropriate services and resources are provided to all clients; attend training courses designed to increase technical competency and further his/her understanding of whole person care topics, and services necessary to support clients and family members according to program expectations; provide simple instruction and coaching/training related to medication management, public transportation, job preparation, continuing education, budgeting, housing, hygiene, and other subjects related to successful independent living and other life skills; provide client support in obtaining, maintaining, and managing outside resources for financial support; manage caseload by maintaining accurate, complete, and legible field notes and activity logs of all clients contact and support provided.     Minimum Qualifications

    MINIMUM QUALIFICATIONS

    Minimum qualifications are used as a guide for establishing the education, training, experience, special skills, and/ or licenses that are required and equivalent to the following.

    Education : Equivalent to completion of twelfth (12th) grade.

    Experience : Six (6) months of experience in Social Services, Behavioral Health, or Human Services field. Level II: Requires one (1)year of experience equivalent to a Behavioral Health Case Manager I.

    Knowledge of: Basic office procedures and practices, including computer usage; principles and practices of effective customer service; correct grammar, spelling, and punctuation.

    Skill/Ability to: Work and communicate effectively with people of various educational and socioeconomic backgrounds by respecting beliefs, interpersonal styles, attitudes, and behaviors of both clients and co-workers; operate contemporary office equipment inclusive of computer, keyboard, and all applicable electronic equipment; learn and apply basic client care practices. Learn and apply basic short-term crisis intervention techniques; gather information through rapport building, observation, and case reviews to identify barriers to treatment; prepare and maintain accurate case records, reports, and correspondence using correct grammar, punctuation, and spelling; develop and maintain cooperative working relationships with agency staff, community groups, and resource agencies; present oral and written reports concisely and clearly to a multidisciplinary treatment team during case staffing meetings; communicate and interact effectively, orally and in writing, with persons of various educational, socioeconomic, and cultural backgrounds; establish and maintain effective interpersonal relationships at all organizational levels and with the public; complete multiple priority projects with strict deadlines; maintain confidentiality of all information and materials.

    License or Certificate: Possession of, or ability to obtain, a valid California driver's license.

    DESIRABLE EMPOLYMENT STANDARDS

    Education: Supplemental coursework in the behavioral, biological, or social sciences, or community college certification in the areas of Human Services, Social Work, or Home Health Aide are desirable.

    Conditions of Employment:
    • Possession of, or ability to obtain, an appropriate, valid California driver's license.
    • Candidates selected will be required to pass a pre-employment drug and alcohol screening. Additionally, a background investigation may also be conducted, which may include a re-investigation every 10 years for some positions.
    • An Employment Eligibility Verification using E-Verify may be required on the first day of employment for some positions.
    • Some job classes may also require a physical exam.

    Benefit Amount: An annualbenefit amountis provided and may be applied towards health insurance premiums (medical, dental, vision, life and long-term disability).This benefit is pro-rated and paid on a pay period basis (24 pay periods).

    Health Insurance: A choice of PPO and HMOmedical plans which include PPOand HMO dental plans andinclude dentalandvision coverage. Dependent coverage is available. Providers include Anthem Blue Cross, Kaiser Permanente, Delta Dental, andVision Services Plan (VSP).

    Retirement:
    The retirement plan is a defined benefit plan administered pursuant to the 1937 Act County Employees Retirement Act and integrated with Social Security. In addition to ordinary retirement benefits, the plan provides disability and death benefits. Retirement contributions are made by both the County and the employee. The County has reciprocity with the State of California, contracting PERS agencies, and all County 1937 Act Retirement Systems.

    Paid Holiday Leave: 12 set days and 1 personal holiday.

    Vacation Accrual:

    2 weeks per year (0-3 years of service)
    3 weeks per year (3-7 years of service)
    4 weeks per year (7-11 years of service)
    5 weeks per year (11+ years of service)
    Limit of 300 hours.

    Sick Leave Accrual: 12 days per year with unlimited accumulation, 50 hours of which may be used toward family sick leave.

    Group Term Life Insurance and AD&D: $10,000; Provided by Standard Insurance Company.

    Disability Insurance:
    Employees are covered by State Disability Insurance. The premium is paid by the employee.

    Deferred Compensation: A voluntary deferred compensation plan is available.
