DescriptionTHE POSITION: Under general direction, provides advanced skilled nursing to patients with highly complex needs as part of the Ambulatory Care Enhanced Care Management Team. The incumbent will provide case management while actively engaging, supporting, and motivating patients in their pursuit of health objectives. Additionally, the role will involve coordinating primary care, behavioral health services, and community-based long-term services and support.
The ideal candidate will have a strong understanding of Enhanced Care Management (ECM) services and their importance in assisting patients with certain health conditions that can be complex, confusing, and hard to manage. Proficiency in managing and tracking vital health data, scheduling various medical appointments, arranging transportation, and ensuring meticulous documentation is essential.
DISTINGUISHING CHARACTERISTICS:The Senior Registered Nurse-Ambulatory Care series is distinguished from other nursing classifications in that it is assigned to ambulatory care programs affiliated with the Ventura County Medical Center and requires that incumbents demonstrate advanced competency skills and knowledge specific to specialty areas (i.e., family care, women's services, pediatrics, infusion/oncology, surgery/orthopedics, etc.). Incumbents are expected to work at the full scope of their licensure providing clinical care with a decreasing amount of supervision commensurate with their experience. The senior level is distinguished by experience and leadership assignments.
PAYROLL TITLE: Senior Registered Nurse - Ambulatory Care
WHAT WE OFFER: - Educational Incentive for completion of: 2.5% for an Associate's degree, 3.5% for a Bachelor's degree, or 5% for a Master's degree.
- Certification Pay: $0.813 per hour based on scheduled work week hours for each qualified certification up to a maximum of five (5) certifications. One of the five certifications may be paid at $2.00 per hour for a National Certification pursuant to provisions in Sec. 805 of the CNA Memorandum of Agreement.
- Continuous County Service Incentive Pay: 1% for 7 years, 1.25% for 12 years and 1.5% for 17 years
- Flexible Credit Allowance: Biweekly contribution towards medical, dental, and/or vision insurance from authorized plans.
- Pension Plan: Participation in the County's defined benefit plan.

AGENCY/DEPARTMENT: Health Care Agency - Ambulatory Care
Senior Registered Nurse-Ambulatory Care is represented by the California Nurses' Association (CNA) and is eligible for overtime compensation. Salary placement will be determined according to the current CNA memorandum of agreement.
The eligible list established from this recruitment may be used to fill current and future Regular (including Temporary and Fixed-term), Intermittent, and Extra Help vacancies for this position only.

Examples Of DutiesDuties may include, but are not limited to the following:
- Assesses and identifies nursing care needs;
- Plans, organizes and implements nursing plans for patients;
- Develops and implements patient and family education programs;
- Establishes and maintains effective working relationships with physicians, patients and co-workers;
- Teaches, mentors and supervises others routinely;
- Participates in the development of clinic policies, procedures, and protocols for patient care;
- Assists in establishing patient care standards and tracks the performance of nursing staff;
- Effectively prepares and presents reports and recommendations both orally and in written;
- Participates in nursing committee activities;
- Maintains current knowledge in specialty areas; and
- Performs other related duties as required.

EDUCATION, TRAINING, and EXPERIENCE
Requires three (3) years of full-time professional registered nursing experience, including two (2) years of full-time experience in an assigned specialty area.
NECESSARY SPECIAL REQUIREMENTS - Must possess and maintain a current, valid license as a Registered Nurse issued by the State of California.
- Must possess a current, valid Basic Life Support (BLS/CPR) certification by first day of employment and maintain it thereafter .
DESIRED - Experience in Ambulatory Care and/or Whole Person Care.
- Experience working with individuals facing substance use challenges and/ or those dealing with mental health issues.
KNOWLEDGE, SKILLS, and ABILITIES:Comprehensive knowledge of: the principles, practices, and techniques used in nursing care at specialty clinics.
Working knowledge of: patient and staff educational methods; quality assurance processes; nursing committee activities; and clinic administrative functions.
Working ability to: provide direction and serve as a resource to others; complete required continuing education programs; work proficiently in emergency and/or stressful situations; maintain confidentiality of patient/client information; effectively maintain a positive working relationship with the medical staff, public, patients, and hospital administration; and communicate effectively, both orally and in writing.
