Behavioral Health and Recovery Services - I MPACT Program There is one (1) full-time position available in the Behavioral Health & Recovery Services Division (BHRS) working on the IMPACT team .
The IMPACT team provides comprehensive assessment, intensive case management, medication management, peer support and other recovery support services that are accessible 24/7 to adults 18 years of age and over. The IMPACT team serves individuals experiencing serious mental illness, co-occurring substance use conditions and/or complex medical issues. In addition, individuals may be experiencing episodes of homelessness, frequent hospitalization, crisis service utilization , and justice involvement . The IMPACT team is leading efforts to operate as an Assertive Community Treatment team, whose core values are rooted in recovery and resiliency, and delivering community-based services in an integrated, holistic and person-centered manner.
Under the direction of the Behavioral Health and Recovery Services Unit Supervisor, the Social Service Worker II will serve as a co-occurring substance use specialist . They will be responsible for providing recovery-focused substance use screening, assessment, counseling and case management services for IMPACT clients, as well as providing consultation and coordinating care with other IMPACT team members, a client’s natural supports and community partners to provide a comprehensive and holistic treatment experience .
This position requires travel throughout Marin County, including providing field-based services .
In addition to the minimum qualifications outlined below, a current Alcohol and Other Drug (AOD) services Certificate required for this BHRS role.
Division of Homelessness and Coordinated Care (2 vacancies) Position 1 - Behavioral Health Bridge Housing :
The County of Marin Health & Human Services Department (HHS) seeks a Social Service Worker II (monolingual) to join the Division of Homelessness and Coordinated Care, with a dotted-line relationship to the Behavioral Health and Recovery Services (BHRS) Division. This fixed-term position, funded through the Department of Health Care Services’ Behavioral Health Bridge Housing (BHBH) grant, is approved through June 30, 2027.
The Social Service Worker II will engage individuals experiencing homelessness who have serious behavioral health conditions, including serious mental illness (SMI) and/or substance use disorders (SUD). The incumbent will independently manage a caseload and provide housing navigation, case management, and stabilization services to help participants obtain and retain permanent housing. Responsibilities include conducting assessments, developing case plans, assisting with housing applications and move-ins, supporting tenancy and landlord relationships, connecting participants to resources and benefits, and maintaining accurate documentation.
This position plays a key role in Marin County’s strategy to reduce homelessness and improve behavioral health outcomes by ensuring that housing is recognized as the foundation for recovery and wellness.
Fixed-Term:
This is a fixed-term position . This position is budgeted through June 30, 2027 . However, there may be opportunities for the hired individual during this time-period to move to a regular, ongoing position as other vacancies or promotional opportunities occur, or the position may be funded and convert to an ongoing position. Fixed-term employment ends when the position’s funding finishes, or the work is completed. Employees in these positions are exempt from the County’s policies regarding lay-off order and reduction-in-force rights.
Position 2 - Homelessness and Coordinated Care:
The Social Service Worker II (monolingual), assigned to the Homelessness and Coordinated Care Division within the Health and Human Services Department (HHS), provides advanced casework and advocacy services to individuals and families experiencing homelessness.
Working independently, the incumbent manages a caseload and delivers housing navigation, case management, and stabilization services to support participants in obtaining and maintaining permanent housing. Cases may come through the Marin Coordinated Entry system, CalAIM , or other pathways. Core responsibilities include conducting assessments, developing individualized service plans, assisting with housing applications and move-ins, fostering positive landlord-tenant relationships, connecting participants to resources and benefits, and maintaining thorough and accurate documentation.
Additional key responsibilities include:
- Conducting comprehensive assessments and developing and implementing individualized service plans.
- Coordinating housing, health, mental health, and community-based supports .
- Applying trauma-informed and client-centered approaches to address complex needs such as housing instability and dual diagnoses.
- Monitoring case outcomes to promote long-term stability and community well-being.
This position requires advanced assessment, intervention, and problem-solving skills, along with the ability to work with minimal supervision. The Social Service Worker II may also mentor junior staff, provide consultation on complex cases, and collaborate with community partners to strengthen service delivery across the county.
This is a regular hire ongoing position.
Planning and Administration Division The Social Service Worker II fixed term position is under the Planning and Administration Division within the Health and Human Services (HHS) Department. This position will be instrumental in implementing and coordinating the Medi-Cal benefit, Enhanced Care Management program (ECM). The program is intended to be a wraparound, high touch service providing client-centered, whole person care coordination services to individuals who meet eligibility requirements for ECM including, but not limited to, those who are currently receiving Behavioral Health and Recovery Services (BHRS), currently experiencing homelessness, and/or transitioning from incarceration from Marin County. This multi-faceted role includes client case management, coordination, and educating the public about services.
Key Duties:
• Outreach and Engagement: Contacts and engages individuals who have been identified as candidates for ECM
• Health Promotion: Provides health promotion and education for clients and their families to engage and help the client manage their care
• Care Plan Development: Develops comprehensive, individualized, person-centered care plans with input from the client and/or their personal support system.
• Enhanced Coordination of Care: Helps clients navigate, connect to, and communicate with physical health, behavioral health, and social service systems ( e.g. programs including MediCal , CalFresh, CalWorks , General Relief, protective services, veteran’s services, and housing, etc.)
• Care Documentation: Prepares and maintains care coordination records and shares information and reports with the health plan, submitting claims and/or invoices as needed.
• Comprehensive Transitional Care: Develops plans for clients who are transitioning out of correctional facility and provides clients with referrals for continued services post-discharge
This role’s care coordination will ensure that clients are connected and engaging in services that will support client’s ability to live independently and thrive in their community . This position will be working closely with other County and external partners ( e.g. BHRS, Public Guardian, Social Security Administration, Public Health, Social Services, medical care providers, Housing agencies, Justice system, and other agencies and programs).
This work requires travel throughout Marin County.
Hybrid Schedule: The Health and Human Services Department has embraced a
hybrid work schedule that considers that some duties of this position may be performed remotely. However, the successful candidate must be able to report to County facilities for meetings, work assignments, and/or disaster service work upon short notice, as needed and deemed appropriate. During the initial probationary period, the successful candidate may be required to work primarily onsite before establishing a hybrid work schedule.
ABOUT YOU Our Highly Qualified Candidate The ideal candidate has experience managing a caseload of clients from diverse and hard-to-reach populations. They should have experience completing clients assessments, developing case plans, fostering engagement and connecting their clients to services. The ideal candidate also possesses effective verbal and written communication skills, strong organizational and excellent time management skills, and the ability to work in a high volume, fast-paced and evolving work environment. The candidate work is able to work both independently exercising considerable judgement and excels in a collaborative and multi-disciplinary team environment. The ideal candidate demonstrates empathy, resilience, and strong problem-solving skills, with the ability to work effectively across systems to support participants on their path from homelessness to housing stability. The ideal candidate provides a welcoming environment for clients at any stage of readiness and should be able to manage crisis situations. The candidate should be able to document complex clinical information quickly, accurately, and concisely in an electronic health record, and utilize this information to develop a care plan.
The BHRS IMPACT ideal candidate has experience providing recovery-oriented substance use assessment, treatment and case management services for individuals with complex co-occurring serious mental illness and substance use conditions, as well as knowledge of applicable evidence-based practices, such as Motivational Interviewing and Assertive Community Treatment.
Division of Homelessness ideal candidate has experience in housing navigation, case management, and supporting individuals with behavioral health needs who are experiencing homelessness. The successful candidate brings:
•Knowledge of social work principles, behavioral health conditions, housing programs, and landlord/tenant rights.
•Skills in engagement, motivational interviewing, case planning and management, and collaboration with landlords, service providers, and community partners.
•Ability to manage a complex caseload independently, conduct home visits, and maintain accurate, timely documentation.
•Cultural humility and the capacity to build trust with individuals from diverse socio-economic and cultural backgrounds.
•A commitment to helping people achieve housing stability as a foundation for health and recovery.
Planning and Administration Division’s ideal candidate has experience providing complex care coordination services to adults and older adults experiencing serious mental illness and/or substance use disorders, and individuals experiencing homelessness, and/or those transitioning from incarceration. They also are able to understand and apply complex regulations in client assessments, can submit treatment authorizations to Managed Care Plans.
