Overview The Licensed Vocational Nurse (LVN) delivers services to the public in accordance with Public Health Standards from the Texas Department of State Health Services and other public health organizations. This position collects and maintains patient information and determines patients' needs as well as the needs of the clinic. The LVN renders public health nursing services to individuals, families, and/or special groups.
Responsibilities • Interpret and review vaccination records and determine vaccine needs based on patient Information
• Administer oral medications/vaccines and injections to patients of all ages
• Utilize EMR/EHR databases to record vaccines and document vaccines given along with vaccination history
• Test patients in a clinic or field setting for communicable diseases
• Provide health screenings for patients
• Serve as back-up coordinator for federal vaccine programs
• Deliver samples to the in-house laboratory for processing, order medication from the Texas Department of State Health Services, maintain medication and refrigerator/freezer temperature logs
• Monitor patients and address medical questions from patients and community members as they arise
• Use clinical response measures in the case of emergency within the clinic and the Public Health District
• Diagnose patients for STI/HIV, discuss treatment plans and referrals, as needed
• Assess the general appearance of clients, elicit medical history, monitor clients for signs of illness at each visit
• Educate and counsel clients with specific health goals or conditions
• Connect to patient navigation services as needed
• Stay up to date on information and guidelines supplied by State and Federal sources
• Create processes to ensure storage, handling, and delivery Is carried out appropriately among clinical staff
• Attend DSHS calls/webinars assigned
• Obtain and maintain CEs as required by licensure and public health programs
• Follow up on vaccine preventable disease reports, providing case management as guided by the Texas Department of State Health Services and reporting information on the State's quarterly report
• Participate in disease surveillance activities, investigate reports of communicable disease
• Enter disease surveillance information into computerized database
• Document diagnosis via EMR/EHR
• Provide support to all Health District clinics and programs as needed including but not limited to outreach and screenings for Health Education, immunizations, STD/HIV, and Tuberculosis
• Maintain records pertaining to medical history, medication, progress notes, labs, consults, Imaging studies, transfer of patients, and other documentation for case management of patients
Position Type and Typical Hours of Work • Non-Exempt - Full-Time
• Non-rotating 40-hour work week, Monday - Friday, 8:00am - 5:00pm
• Must be available to work additional hours as needed
• Flexibility to work evenings, weekends, and holidays is a schedule requirement
• In the event of an emergency, employees are required to work to provide for the safety and well-being of the public, including the delivery and restoration of vital services

Minimum Qualifications • Associate's degree (AA/AS); or 2-year technical certificate
• Two (2) years of experience
Licenses and Certifications Required • A valid driver's license is required; successful out-of-state candidates must be able to obtain a valid Texas driver's license within 90 days of hiring
• Board of Nursing LVN Certification with current license
Required within six (6) months of hire • Basic Life Support (BLS) training in CPR/First Aid

Basis of Rating A recruitment consultant will evaluate all applications against the posted qualifications. The city may also conduct additional skill assessment tests, in addition to the panel interview
